After booking an appointment, please download, print and fill out the Medical information / Consent to Treatment form.
This form will need to be completed before your massage, or body works, can be started.
Make sure all information is up to date and correct, and have it ready and available at your appointment.
If you do not have access to a printer, no worries, I will have some available for you to fill out.
